Overall, Sheran said that its helpful to think of the option to switch from TDF to TAF as a choice guided by input from a provider.
My opinion in terms of prescribing is to be patient-centered, he said. TAF is another option, and there doesnt have to be dogmatic black and white rules about every aspect of medication choice. How Do You Take Truvada And Tivicay

Monica Gandhi from the University of California at San Francisco presented follow-up findings on kidney function in the pivotal iPrEx trial, the first to show that once-daily Truvada PrEP was highly effective for gay and bisexual men and transgender women who took it consistently. In an open-label extension , in which everyone knew they were taking active Truvada, no one who took the pills at least four times a week became infected with HIV.
The iPrEx team looked at the relationship between tenofovir and emtricitabine concentrations in hair samples which reflect cumulative drug exposure and kidney function over time in iPrEx OLE. Hair samples were collected, serum creatinine was measured and estimated glomerular filtration rate was calculated every 12 weeks.
Of the more than 1000 iPrEx OLE participants, hair drug levels and creatinine measurements were available for 220 people. A majority of participants in this international study were Latino or mixed race/ethnicity and the median age was 29 years . At baseline, the mean creatinine level was 0.89 mg/dl and the median estimated eGFR was 112 ml/min .
Establishing thresholds of tenofovir/emtricitabine exposure that minimise adverse events while maximizing efficacy are essential to the real-world roll-out of PrEP, the researchers wrote.

Participants returned to the clinic for follow-up at a start, mid-point , and end-point visit for safety and adverse event monitoring, symptom-directed physical exams, and rapid HIV testing.

HIV can pass to others in blood, semen, and breast milk, as well as in vaginal and rectal fluids. You can prevent HIV from passing to others by:
- using condoms every time you engage in sexual activity
- choosing sexual activities, such as oral sex, that are less likely to transmit HIV than anal or vaginal sex
- getting regularly tested and treated for other sexually transmitted infections

Rise In Lipid Levels With Taf
There are a number of studies showing that TDF-containing regimens oftentimes improve lipid levels by decreasing total cholesterol, LDL cholesterol and triglycerides, while TAF-containing regimens increase these levels. Higher levels of total cholesterol and LDL cholesterol can promote plaque buildup in arteries, which can lead to coronary artery disease, stroke and other health problems.
In the DISCOVER study, which compared Descovy to Truvada for PrEP, triglyceride and LDL cholesterol levels increased at 48 weeks for people taking Descovy compared to no change or a decrease in people taking Truvada. Decreases were seen in total cholesterol and HDL cholesterol .
Other studies have compared the lipid levels of people living with HIV taking TAF-containing or TDF-containing treatment regimens or switching from a TDF-containing regimen to a TAF-containing regimen.
Anobservational study conducted in Spain compared the lipid profiles of people on Genvoya or Stribild for HIV treatment over 48 weeks.
Lipid levels increased from baseline among people taking Genvoya while remaining stable among people taking Stribild after 48 weeks of treatment. A higher percentage of people taking Stribild also had total cholesterol levels and LDL levels out of range after 48 weeks of treatment. Six people in the study discontinued Genvoya due to changes in lipid levels, and lipid levels returned to normal levels after returning to treatment that included TDF.
