Hormone Therapy For Prostate Cancer

Hormone therapy is also called androgen suppression therapy. The goal is to reduce levels of male hormones, called androgens, in the body, or to stop them from fueling prostate cancer cells.

Androgens stimulate prostate cancer cells to grow. The main androgens in the body are testosterone and dihydrotestosterone . Most androgens are made by the testicles, but the adrenal glands as well as the prostate cancer itself, can also make a fair amount.

Lowering androgen levels or stopping them from getting into prostate cancer cells often makes prostate cancers shrink or grow more slowly for a time. But hormone therapy alone does not cure prostate cancer.

Active Surveillance And Watchful Waiting

If prostate cancer is in an early stage, is growing slowly, and treating the cancer would cause more problems than the disease itself, a doctor may recommend active surveillance or watchful waiting.

Active surveillance. Prostate cancer treatments may seriously affect a person’s quality of life. These treatments can cause side effects, such as erectile dysfunction, which is when someone is unable to get and maintain an erection, and incontinence, which is when a person cannot control their urine flow or bowel function. In addition, many prostate cancers grow slowly and cause no symptoms or problems. For this reason, many people may consider delaying cancer treatment rather than starting treatment right away. This is called active surveillance. During active surveillance, the cancer is closely monitored for signs that it is worsening. If the cancer is found to be worsening, treatment will begin.

ASCO encourages the following testing schedule for active surveillance:

  • A PSA test every 3 to 6 months

  • A DRE at least once every year

  • Another prostate biopsy within 6 to 12 months, then a biopsy at least every 2 to 5 years

Treatment should begin if the results of the tests done during active surveillance show signs of the cancer becoming more aggressive or spreading, if the cancer causes pain, or if the cancer blocks the urinary tract.

How Long Does It Take To Work

Xtandi is used to treat certain types of prostate cancer that no longer improves with typical medications or surgical treatments that work by lowering testosterone levels.

Xtandi works by helping to stop cancer cells from growing. How quickly it works depends on how progressed your cancer is. For this reason, its hard to say when the drug may start to work for you.

    Xtandi comes as capsules that are taken by mouth. Its available in one strength: 40 mg.

    The typical dosage of Xtandi is 160 mg taken by mouth once each day. It can be taken with or without food.

    Zytiga comes as tablets that are taken by mouth. Its available in two strengths: 250 mg or 500 mg.

    How Long Does Chemo Stay In The Body And Bloodstream

    The benefits of chemotherapy can be long-lasting. This is crucial in fighting cancer, but it raises the question of how long these drugs stay in the bloodstream.

    These powerful drugs are broken down by the kidneys and liver and leave your body in urine, stool, vomit, vaginal fluids, and sweat.     Examples of drugs that decrease the level of Xtandi in your body, and should be avoided if possible, include:

    What Happens When You Take Expired Pills?

    In addition to treating prostate cancer, Lynparza is also used to treat certain other conditions in adults. Lynparza treats the following cancers by blocking the action of specific enzymes . Blocking these enzymes damages cancer cells, causing them to die.

    Lynparza for ovarian cancer

    Lynparza is used to treat advanced ovarian cancer that is BRCA-positive* in adults when the cancer:

    • has been treated with three or more chemotherapy drugs , or
    • is also homologous recombination deficiency-positive

    HRD-positive refers to a specific kind of cancer caused by an abnormal BRCA gene. For this purpose, Lynparza is used as ongoing treatment along with bevacizumab after your cancer has improved following treatment with first-line chemotherapy made from platinum.

    Its also used as maintenance therapy for the following:

    • Cancer thats improved following treatment with first-line chemotherapy made from platinum.
    • Recurrent ovarian cancer that has completely or partially responded to chemotherapy made from platinum.

    Treatment To Lower Testicular Androgen Levels

    Androgen deprivation therapy, also called ADT, uses surgery or medicines to lower the levels of androgens made in the testicles.

    Orchiectomy

    Even though this is a type of surgery, its main effect is as a form of hormone therapy. In this operation, the surgeon removes the testicles, where most of the androgens are made. This causes most prostate cancers to stop growing or shrink for a time.

    This is done as an outpatient procedure. It is probably the least expensive and simplest form of hormone therapy. But unlike some of the other treatments, it is permanent, and many men have trouble accepting the removal of their testicles. Because of this, they may choose treatment with drugs that lower hormone levels instead.
