Hormone Therapy For Prostate Cancer

Jump to a section

Hormone therapy is also called androgen suppression therapy. The goal is to reduce levels of male hormones, called androgens, in the body, or to stop them from fueling prostate cancer cells.

Androgens stimulate prostate cancer cells to grow. The main androgens in the body are testosterone and dihydrotestosterone . Most androgens are made by the testicles, but the adrenal glands as well as the prostate cancer itself, can also make a fair amount.

Lowering androgen levels or stopping them from getting into prostate cancer cells often makes prostate cancers shrink or grow more slowly for a time. But hormone therapy alone does not cure prostate cancer.

Active Surveillance And Watchful Waiting

If prostate cancer is in an early stage, is growing slowly, and treating the cancer would cause more problems than the disease itself, a doctor may recommend active surveillance or watchful waiting.

Active surveillance. Prostate cancer treatments may seriously affect a person’s quality of life. These treatments can cause side effects, such as erectile dysfunction, which is when someone is unable to get and maintain an erection, and incontinence, which is when a person cannot control their urine flow or bowel function. In addition, many prostate cancers grow slowly and cause no symptoms or problems. For this reason, many people may consider delaying cancer treatment rather than starting treatment right away. This is called active surveillance. During active surveillance, the cancer is closely monitored for signs that it is worsening. If the cancer is found to be worsening, treatment will begin.

ASCO encourages the following testing schedule for active surveillance:

  • A PSA test every 3 to 6 months

  • A DRE at least once every year

  • Another prostate biopsy within 6 to 12 months, then a biopsy at least every 2 to 5 years

Treatment should begin if the results of the tests done during active surveillance show signs of the cancer becoming more aggressive or spreading, if the cancer causes pain, or if the cancer blocks the urinary tract.

Treatment By Stage Of Prostate Cancer

Different treatments may be recommended for each stage of prostate cancer. Your doctor will work with you to develop a specific treatment plan based on the cancers stage and other factors. Detailed descriptions of each type of treatment are provided earlier on this same page. Clinical trials may also be a treatment option for each stage.

    Early-stage prostate cancer

Early-stage prostate cancer usually grows very slowly and may take years to cause any symptoms or other health problems, if it ever does at all. As a result, active surveillance or watchful waiting may be recommended. Radiation therapy or surgery may also be suggested, as well as treatment in clinical trials. For those with a higher Gleason score, the cancer may be faster growing, so radical prostatectomy and radiation therapy are often recommended. Your doctor will consider your age and general health before recommending a treatment plan.

ASCO, the American Urological Association, American Society of Radiation Oncology, and the Society of Urologic Oncology recommend that patients with high-risk early-stage prostate cancer that has not spread to other areas of the body should receive radical prostatectomy or radiation therapy with hormonal therapy as standard treatment options.

    Locally advanced prostate cancer

Watchful waiting may be considered for older adults who are not expected to live for a long time and whose cancer is not causing symptoms or for those who have another, more serious illness.

How Long Does Chemo Stay In The Body And Bloodstream

The benefits of chemotherapy can be long-lasting. This is crucial in fighting cancer, but it raises the question of how long these drugs stay in the bloodstream.

These powerful drugs are broken down by the kidneys and liver and leave your body in urine, stool, vomit, vaginal fluids, and sweat. Some drugs leave your body quickly, others may take a few days, with each passing day, the amount of drug leaving your body after treatment is less. Most chemotherapy drugs are cleared from body waste in about two days. A few drugs may take as long as seven or more days to be cleared.

A variety of factors can influence how long it takes for the drugs to leave your body. The presence of other medicines in your system, your age , and how well your liver and kidneys able to process the drugs.

These drugs kill cancer cells in your body, so your body waste may be dangerous to others around you. Special care should be taken to reduce exposure to small children and pregnant women. Caregivers should wear gloves when handling body waste. The person receiving the chemo could use of a separate bathroom if possible. If there is a spill, clean immediately.

Since chemotherapy effects all the cells in the body, it can have a wide range of side effects on your bodys organs. Each drug is unique, but these are some common side effects:

Your doctor can give you a range of how long any side effect is likely to last.

Treatment To Lower Androgen Levels From The Adrenal Glands

LHRH agonists and antagonists can stop the testicles from making androgens, but cells in other parts of the body, such as the adrenal glands, and prostate cancer cells themselves, can still make male hormones, which can fuel cancer growth. Some drugs can block the formation of androgens made by these cells.

Abiraterone blocks an enzyme called CYP17, which helps stop these cells from making androgens.

Abiraterone can be used in men with advanced prostate cancer that is either:

    • High risk
    • Castrate-resistant

This drug is taken as pills every day. It doesnt stop the testicles from making testosterone, so men who havent had an orchiectomy need to continue treatment with an LHRH agonist or antagonist. Because abiraterone also lowers the level of some other hormones in the body, prednisone needs to be taken during treatment as well to avoid certain side effects.

Ketoconazole , first used for treating fungal infections, also blocks production of androgens made in the adrenal glands, much like abiraterone. It’s most often used to treat men just diagnosed with advanced prostate cancer who have a lot of cancer in the body, as it offers a quick way to lower testosterone levels. It can also be tried if other forms of hormone therapy are no longer working.

Ketoconazole also can block the production of cortisol, an important steroid hormone in the body, so men treated with this drug often need to take a corticosteroid .

    Xtandi And Other Medications

Below are lists of medications that can interact with Xtandi. These lists do not contain all the drugs that may interact with Xtandi.

Before taking Xtandi, talk with your doctor and pharmacist. Tell them about all prescription, over-the-counter, and other drugs you take. Also tell them about any vitamins, herbs, and supplements you use. Sharing this information can help you avoid potential interactions.

If you have questions about drug interactions that may affect you, ask your doctor or pharmacist.

Drugs that decrease Xtandi levels in your body

Xtandi is metabolized by an enzyme called CYP3A4. Certain other drugs increase the amount of this protein in your body. Taking those drugs with Xtandi can decrease the level of Xtandi in your body. This can reduce how effective Xtandi is in treating your condition.

Examples of drugs that decrease the level of Xtandi in your body, and should be avoided if possible, include:

    • certain antivirals, such as nevirapine
    • certain antibiotics, such as:
  • phenobarbital
  • oxcarbazepine
  • Many drugs other than those listed are also metabolized by the CYP3A4 enzyme. Talk with your doctor or pharmacist about all medications youre taking with Xtandi. Your doctor or pharmacist can help keep your risk of drug interactions low.

Drugs that increase Xtandi levels in your body

Examples of drugs that increase the level of Xtandi in your body, and should be avoided if possible, include:

  • certain asthma medications, such as zafirlukast
  • sorafenib
  • erlotinib

What Happens When You Take Expired Pills?

In addition to treating prostate cancer, Lynparza is also used to treat certain other conditions in adults. Lynparza treats the following cancers by blocking the action of specific enzymes . Blocking these enzymes damages cancer cells, causing them to die.

    Lynparza for ovarian cancer

Lynparza is used to treat advanced ovarian cancer that is BRCA-positive* in adults when the cancer:

    • has been treated with three or more chemotherapy drugs , or
    • is also homologous recombination deficiency-positive

HRD-positive refers to a specific kind of cancer caused by an abnormal BRCA gene. For this purpose, Lynparza is used as ongoing treatment along with bevacizumab after your cancer has improved following treatment with first-line chemotherapy made from platinum.

Its also used as maintenance therapy for the following:

    • Cancer thats improved following treatment with first-line chemotherapy made from platinum.
    • Recurrent ovarian cancer that has completely or partially responded to chemotherapy made from platinum.

Before prescribing Lynparza, your doctor will order a test to determine if your ovarian cancer has an abnormal BRCA gene that may respond to Lynparza.

    * This means the cancer has an abnormal change in the BRCA gene.

    Lynparza for breast cancer

Lynparza is used in adults to treat breast cancer with the following characteristics:

Treatment To Lower Testicular Androgen Levels

Androgen deprivation therapy, also called ADT, uses surgery or medicines to lower the levels of androgens made in the testicles.

Even though this is a type of surgery, its main effect is as a form of hormone therapy. In this operation, the surgeon removes the testicles, where most of the androgens are made. This causes most prostate cancers to stop growing or shrink for a time.

This is done as an outpatient procedure. It is probably the least expensive and simplest form of hormone therapy. But unlike some of the other treatments, it is permanent, and many men have trouble accepting the removal of their testicles. Because of this, they may choose treatment with drugs that lower hormone levels instead.

Some men having this surgery are concerned about how it will look afterward. If wanted, artificial testicles that look much like normal ones can be inserted into the scrotum.

    LHRH agonists

Luteinizing hormone-releasing hormone agonists are drugs that lower the amount of testosterone made by the testicles. Treatment with these drugs is sometimes called medical castration because they lower androgen levels just as well as orchiectomy.

With these drugs, the testicles stay in place, but they will shrink over time, and they may even become too small to feel.

    • Leuprolide
    • Triptorelin
    • Leuprolide mesylate

    LHRH antagonists

    Possible side effects

Many side effects of hormone therapy can be prevented or treated. For example:

Does Lynparza Cause Weight Gain Hair Loss Or Bloating

Weight gain wasnt a side effect reported in Lynparzas studies.

Bloating was a side effect reported in these studies among people who took Lynparza to treat ovarian cancer.

Hair loss wasnt a side effect originally reported in Lynparzas studies. But since the drug was approved for use, it has been reported by people taking Lynparza for breast cancer.

If you notice symptoms of bloating or hair loss while taking Lynparza, let your doctor know. They may be able to recommend a treatment for the side effect.

Can Chemo Reverse Resistance To Zytiga And/or Xtandi Or Does It Play Any Role In That Scenario

What causes Hair Loss and How to Regrow it

Dr. Graff: I dont know if it can reverse it. I have seen data showing that, if youre on Xtandi and the cancer cells become resistant to that, then if you put a patient on chemo, some of those cells that arent resistant to Xtandi might come back, and it might be reasonable to re-treat it then. Thats not carved in stone.

Recovery from cancer is not always possible. If the cancer cannot be cured or controlled, the disease may be called advanced or terminal.

This diagnosis is stressful, and for some people, advanced cancer may be difficult to discuss. However, it is important to have open and honest conversations with your health care team to express your feelings, preferences, and concerns. The health care team has special skills, experience, and knowledge to support patients and their families and is there to help. Making sure a person is physically comfortable, free from pain, and emotionally supported is extremely important.

People who have advanced cancer and who are expected to live less than 6 months may want to consider hospice care. Hospice care is designed to provide the best possible quality of life for people who are near the end of life. You and your family are encouraged to talk with the health care team about hospice care options, which include hospice care at home, a special hospice center, or other health care locations.
