Finasteride was initially released on the market in 1992, in a 5mg tablet strength which was FDA approved under the brand name Proscar for use in men with benign prostatic hyperplasia. 5 years later, in 1997, another Finasteride-containing medication was released under the brand name Propecia.

Propecia is only available in the 1mg tablet strength, and its FDA-approved indication is only for androgenic alopecia in men, not anything prostate related.

So, why is the same ingredient approved for two separate and seemingly diverse indications?

It all comes down to strengths and Dihydrotestosterone .DHT is an androgen that develops from testosterone, and this hormone in surplus has been found to be a culprit in both prostate issues and the shrinking of the hair follicles in the scalp causing androgenic alopecia.

Finasteride help remedy these situations by blocking the enzyme which helps form DHT from testosterone, thus reducing the DHT levels in the body.

The path to the development of finasteride took several decades and chance discoveries.

In 1942, James B. Hamilton of the Yale University School of Medicine that correlated male pattern hair loss with male sex hormones. By examining men who had failed to reach sexual maturity due to castration at a pre-pubescent age, Hamilton found that none of them suffered from the typical signs of male pattern baldness and hair loss. In men who were castrated during their adolescent years, most retained a full head of hair, while some had slight hair loss.

The next step wouldnt come for the next three decades. In 1974, Julianne Imperato-McGinley of Cornell Medical College in New York presented her results documenting the traits of a group of intersex children in the Dominican Republic who presented as female at birth, but began to develop male traits and genitalia at the age of 12. Known as guevedoces, Imperato-McGinley found that the children shared a rare genetic mutation that impaired the expression of the 5-reductase enzyme. Given that 5-reductase converts testosterone to the potent androgen DHT, it was posited that the absence of DHT contributed to the delayed development of male sexual traits. As one of her observations made in the presentation, Imperato-McGinley noticed that among the guevedoces, the prostate remained small.

Study: Finasteride 5 Mg/day Treatment Of Patterned Hair Loss In Normo

Finasteride is a drug thats available in various dosages. And while its important to use the lowest dose possible so as to reduce the risk of adverse effects, researchers have studied various dose strengths on women to determine true efficacy of the drug .

In one particular study, published in 2013, researchers wanted to see how effective a 5mg/day dose of finasteride would be on postmenopausal women with AGA.

A total of 40 women participated in this study, and all of them received a daily dose of 5mg finasteride for a 18 months.

The efficacy of the treatment was evaluated by patients satisfaction, as well as global photograph assessment by two independent dermatologists.

After 6 months, 22 patients referred significant improvement, 12 referred moderate improvement, and 6 referred no improvement. These were somewhat in line with the photograph assessments of which the dermatologists determined that 8 patients showed no improvement, 16 showed moderate improvement, and 16 showed significant improvements at the 6th month

A slight improvement was observed from the 6th month to 12 and 18 months.

While the results were not as objective as desired, they do strongly suggest that 5mg finasteride is effective and safe for the treatment of female AGA in postmenopausal women in the absence of clinical or laboratory signs of hyper-androgenism.
